The player takes on the role of a tooth fairy-like creature that enters people's homes at night to collect their teeth. The game begins with a map of a neighborhood, where each house represents a potential target. Hovering over a house reveals information about the occupant, such as their age (child, teen, adult, elder) and various traits (e.g., tough, brave, coffee addict, heavy sleeper). Once a house is selected, the player is presented with a close-up of the sleeping person's mouth. The objective is to extract their teeth using a variety of tools. The player must manage three bars: Lucidity: This indicates how close the person is to waking up. Pain: This shows the level of pain being inflicted. Fear: This represents the person's level of fear. The player uses different tools to extract teeth, including a hook to pry open the mouth, a drill-like tool to loosen teeth, and forceps to pull them out. Different types of teeth are collected, such as "decayed," "baby," "adult," "cracked," "dirty," "aged," "crowned," "gold," "silver," "glass," and even rare and unsettling types like "unearthly key tooth," "watcher tooth," and "sharp tooth." Successfully collecting all types of teeth is the main goal, as indicated by a completion percentage and a collection screen showcasing the different teeth acquired.
